The complimentary webinar, "Proven Strategies to Reduce Readmissions and Drive Patient Engagement," will be held on Monday, Sept. 29 at 11 a.m. PT/2 p.m. ET.

These key experience improvement leaders will detail how they redesigned nurse leader rounding, discharge communication and patient follow-up calls to improve care team coordination and patient engagement. They will also discuss how evidence-based approaches helped their organizations achieve measurable results such as:

  • Standardized workflow and data capture to improve efficiency
  • Reduced readmission rates by 20 percent
  • Increased Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) discharge domain rating up to the 95th - 97th percentile

The panel, moderated by Vocera Chief Nursing Officer Rhonda Collins, includes Abby Nguyen, chief nursing officer at Genesis HealthCare System; Keri Brookshire-Heavin, vice president of patient care services and chief nursing officer at Phelps County Regional Medical Center; and Sunitha Sastry, director of patient improvement and innovation at The University of Chicago Medicine.
